404 Error when add a new user or group

I am on 9.5.2. For the past couple month, I can't add a group or user in a normal way. I always get "HTTP Error 404 - File or directory not found" message. Any one had this issue?

Not seen it before but at a guess it sounds like you could be using CAM security and the Cognos service isn't running or TM1 isn't set up to correctly link to it but if that was the case I'd also expect to see issues with logging in.
If you aren't using CAM security what is the AllowImportCamClients config parameter set as in your tm1p.ini file?
