Wierd behaviour of Action Forms

Client OS: XP

I have created some reports using Action forms and sometimes I face the following wierd behaviours by Action Forms.

1) Sometimes the Show/Hide Format Area menu options in Perspectives do not do anything.
2) Sometimes the data shown in the Action Forms is not correct (all the columns show the same data which is not true)

After generating the Action Form in Cube Viewer I am changing some colour formats, inserting logo and adding a couple of lines below the [End Format Range] row.

Is there anything which I should do to avoid this behaviour or what I am doing wrong here? Please guide me. Thanks
