Just as a follow-up and in case anyone else runs into this problem, at our company, we found that this issue was the result of an interaction with Cisco's CUPC (Cisco Unified Personal Communicator) software. It's still a problem in 9.5.2. If it's been on the table this long, it must not be much of a priority.
